LIST OF REQUIRED DOCUMENTS:
- If a correction in name is required, you may submit any standard proof of name such as your driving license, PAN Card or Passport Copy
- In case of a change in surname, kindly submit a copy of your marriage certificate.
- In case of a complete name change, we additionally require a gazette notification and newspaper cutting.
